Why Time To Care Exists

Meet Karen Norman-Smith (SRN/M)

With prior years as a Senior South African Registered Nursing Sister managing various medical departments, I founded Time To Care in 2019.

I had seen the need for good quality home-based care, centred around one’s individual needs.

Knowing the load a family bears, finding, vetting and managing carers can be overwhelming for the family.

Time To Care places caregivers with the addition of genuine, continual supervision and care plan management. I stay in constant contact with the placed caregivers and communicate with the family as needed.

Time To Care’s goal is to protect your loved one’s dignity and manage their care plan. Adapting when and where needed for their continuous comfort.

Our Services

What we offer

Caregiver Supervision

Based on your initial assessment, an individual care plan will be devised, discussed, and agreed upon with you in accordance with your required care hours. The care plan will be created to align with the medical treatment prescribed by your medical personnel.

Based on your care plan, a caregiver will be assigned to you. Ongoing communication via reports, cell communication and regular home visits to ensure continual supervision of your loved one’s care and adaptation of the care plan as necessary

Post Operative Home-Care

It has become the norm to reduce a patient’s hospital stay wherever possible. Although stable, many find they need home-care assistance whilst continuing their recovery journey at home.

This is where Time To Care fits in to provide home-based care and assist the patient on their recovery journey. This is done through:

Medical & Complex Recovery Support

  • Post-Operative Support: Professional home care support post-op for those requiring general assistance with daily needs while their health and strength improve back to normal.

This may require dealing with and adapting to a possible disability requiring short to long-term care assistance.

  • Advanced Nursing: Expert management of Hickman and PICC lines, IV antibiotics, and PEG feeding and care.

  • Catheter & Wound Care: Meticulous care for suprapubic catheters and basic wound management with an aseptic technique approach.

  • Pressure Care: A proactive “prevention is better than cure” strategy for bedridden or mobility-impaired patients.

Live In Care

For patients who require full-day care, the caregiver is required to live on the property. Time To Care will arrange the care giving team, taking into account care rotations to ensure we maintain the highest levels of care.

Dignified Ageing & Chronic Care

    • Cognitive Support: Specialised care for Alzheimer’s and dementia, focused on preserving a sense of self and daily routine.

    • Personal Hygiene (required with all forms of care): Always taking into account independent ability and encouragement where able. Assistance with bathing, dressing, and grooming. This is handled by the caregiver with dignity, integrity, compassion and respect.

    • Pressure Care: A proactive “prevention is better than cure” strategy for bedridden or mobility-impaired patients.

    • Medication & Record Keeping: Detailed record-keeping of all prescribed medication, and updating of any changes.

    • Nutrition: Nutrition plays an important role in our body’s healing and health maintenance. Whatever the health care need, Time To Care’s caregiver will follow nutrition plans as designed from the initial assessment requirements. Adapting to any medical or personal requirements going forward.
    • Companionship: Companionship is part and parcel of any caregiver’s care. Encouraging and assisting with the social and emotional welfare of your loved one.

Whether it’s reading aloud, styling one’s hair, home manicure or pedicure, or facilitating Zoom and WhatsApp calls with family and friends. Assisting with hobbies, daily mobility, walks and assisting and encouraging social interactions.

Literally a friendly extra hand, ear, support and someone to communicate with.

Testimonials For Time To Care

  • Time to Care provides holistic eldercare that supports medical needs, personal care, and social well-being—promoting dignity, independence, and quality of life as people age. My widowed mother began experiencing cognitive decline after the age of 85 and has been under the care of Time to Care for the past eight years. Her care has been overseen by Sister Karen Norman Smith founder and operator of Time to Care. As a highly experienced nursing sister with an extensive career managing hospital departments and multidisciplinary teams. Her leadership, clinical judgment, and ability to match her staff’s skills precisely to my mother’s evolving medical and personal needs have been exceptional.

The nursing team assigned to my mother has consistently demonstrated professionalism, discipline, and reliability. Their meticulous daily clinical recordkeeping has enabled early identification of health changes that could lead to medical events which are often averted. Time to Care maintained rigorous safety and infection-control standards, successfully protecting my mother during COVID-19 and other infectious periods.

Medical coverage has been comprehensive, including close coordination with her general practitioner, specialist physicians, and a clearly defined urgent care plan with the local hospital. Her care has included medication management and therapeutic interventions for both chronic and episodic illnesses. This proactive primary care approach has repeatedly prevented hospital admissions, managing conditions ranging from minor pulmonary infections to more serious medical concerns within the home whenever safely possible.

In the early stages of care, Time to Care placed permanent, reliable nursing staff in my mother’s home. Over time, they became trusted members of our family. They adapted seamlessly to her lifestyle and daily routine, while remaining sensitive to the dynamics of a mature, empty-nest household. They have facilitated Zoom calls with former colleagues as well as online birthday parties with family located overseas.

As my mother’s physical abilities declined, the team worked closely with her domestic staff to ensure that safety measures were progressively implemented, cleanliness standards were maintained daily, and her personal independence was preserved for as long as possible. Her personal hygiene and home environment were managed with care, consistency, and respect.

Beyond clinical care, the Time to Care staff supported my mother’s social life, assisting her in maintaining connections with friends, neighbours, and her cherished church community. They ensured she was appropriately dressed, well-presented, and confident when receiving visitors, even washing and styling her hair for special occasions. These thoughtful details contributed greatly to her sense of self-worth.

As she transitioned into full-time incontinence, her dignity was carefully preserved through attentive intimate care, infection prevention, wound management, and long-term pressure and bedsore prevention.

Time to Care management of my mother’s integrated primary health care extended to collecting medications, managing prescription renewals, coordinating laboratory testing, and accompanying her to medical appointments. They consistently advocated on her behalf and maintained regular communication with her specialists to prevent short- and medium-term health crises.

My mother’s chronic and age-related conditions were managed through a combination of clinical oversight and therapeutic interventions aimed at preserving her quality of life. Her health status—whether stable, improving, or declining—was continuously monitored through detailed documentation. This ongoing surveillance proved invaluable to medical professionals in assessing her condition and recommending appropriate treatment over time.

Time to Care’s commitment, professionalism, and compassionate approach have allowed my mother to age safely and with dignity in her own home. I remain deeply grateful for the exceptional standard of care they have provided throughout every stage of her aging journey.

  • When our son was three years old he contracted a potentially fatal drug-resistant strain of non-tuberculosis mycobacterium in his inner, middle ear and mastoid area which required prolonged intravenous treatment. During Covid, we were given the option of 2-3 months of hospital-based care or managing his treatment at home with a Hickman line directly into his heart. We chose to treat him at home with the support of Karen from Time to Care.

Due to the nature of treatment and the risks associated with a Hickman line, his treatment regimen was complex, requiring six-hourly sterilisation and flushing of the line, the administration of multiple IV antibiotics, and weekly dressing changes for a period of approximately three months.

Karen provided dedicated, attentive and professional support throughout this period coming to our home, day and night to support this treatment. With a comprehensive knowledge and understanding of the risks and prototcols associated with complex treatment of this nature, Karen was unwavering in her application of these throughout the treatement program inspiring confidence in and directly contributing to a successful recovery.

We are extremely grateful to Karen for her meticulous and dedicated care during that time and would recommend her without hesitation.

  • In 2025, I under went a surgical procedure that required quite a bit of bed rest and nursing. I decided to spend the time in my home rather than in a step down facility and used Karen Smith’s agency to make the arrangements. She came to take a medial history before I even went into hospital and checked that my home was suitable. She provided advice on what I needed to have and gave me insight into what I could expect. She was there to settle me in post op, and left me to the care of her trained carers. They are a delightful team, kind, caring and they helped me get through a really challenging time. Karen popped in to regular visits and to read the charts and make sure I was progressing. I recommend her services to those needing home caregivers.”

  • I cannot thank Karen enough for her care with my mom’s suprapubic catheter. She is incredibly gentle and strict about cleanliness, which kept my mom comfortable, relaxed, and safe. It is such a relief to have her doing this at home rather than facing the stress of the hospital outpatient emergency room. Karens support has been absolutely limitless.

  • Time to Care provided an incredible service to our family. They took away all the stress of having to find, vet, and schedule carers which we had already tried doing ourselves. Karen’s attention to detail and personal check ins were so welcomed and appreciated by us all and she even helped to manage difficult dynamics that arose which were above and beyond our expectations. We would definitely recommend Karen and her team to anyone needing home care.”

  • There is no one I would rather take care of my loved one in their last days than Karen. She and her ‘Time to Care’ team blend compassion and kindness with the utmost professionalism. They did everything to minimise my Mum’s suffering and maintain her dignity. There were so many small acts of caring that meant so much. One day I mentioned that my Mum’s nails had never not been manicured as far back as I could remember. (She was a physio.) When I came back, the Carer had painted her nails. Every day a fresh, bright scarf adorned her bed. Not only did they give my Mum the best care, they supported me through one of the hardest times of my life. I cannot thank them enough. ❤️

  • I, John Michael Els am hereby privileged to write a few words with regard to Karem Norma-Smith who is the owner of Time To Care. Time To Care is a home-based care and support establishment with a tailored support system which is undoubtedly the best that anyone can find.

After lots of research and many, many phone calls to find anyone suitable to care for my now departed wife who suffered from Amyotrophic Lateral Sclerosis (ALS), a neurological disorder, oxygen deficiency and anxiety. I was blessed in managing to contact Karen for assistance.

At the outset Karen was extremely efficient with a professional and friendly approach in making an immediate appointment to come to my holiday home in Ballito within 24 hours of first contact.

Karen met my wife Marilynn and myself evaluating her condition and symptoms as well as checking on any health and safety issues that may be relevant to our situation.

She immediately summed up what was required making a few suggestions like bringing in two very likeable Carers that fitted in perfectly as live-in Carers two weeks on, and two weeks off. We took them in as “family” and Marilynn and I felt blessed with them, They were extremely passionate, friendly honest and very efficient in dealing with Marilynn’s continuous daily monitoring of her medication and dietary intake. Karen insisted on hourly and daily reports physically checking these reports whenever she arrived daily to check on my wife.

Karen has no doubt the dedication, compassion and empathy that goes far above and beyond caring for her patients. We have since kept in close contact nearly three years after Marilynn passed on. She, still maintains contacts with me to ensure she can give me my flu injection annually filling in all the necessary Discovery documentation required so that the Medical Aid pays on my behalf.

Karen’s professional care and clinical expertise improved Marilynn’s quality of life and in fact may have assisted her dignity, recovery and hope to improve her lifespan.

It is not only her passion to assist with medical care, but Karen became and friend of the family from the time we met some maybe three or four years ago during the latter stages of Covid. Her unwavering patience continuously taking time out of her busy daily schedule coming in after hours to check up and give advice where needed. Karen always made Marilynn feel totally comfortable calming her anxiety attacks on the occasions when I was sometimes at a loss of what to do next.

I can further attest to the fact that all my family, friends and close neighbors will be able to confirm our total gratitude that I always praised Karen for the incredible assistance she provided to both Marilynn and myself. We know that God will bless her for the incredible passion and care she projects to her patients.
